This form is only to be used when nominating an individual(s) for one of the "Special Awards" ie:

Prosecutor of the Year, Sheriff of the Year, A/P of the Year, Lifetime Achievement, NJ State Narcotic Officer, North Narc Officer of the Year, Central Narc Officer of the Year, South Narc Officer of the Year, Undercover Officer of the Year, K-9 Officer of the Year, Intel Analyst of the Year, Trainer of the Year, Valor Award, Narcotic Unit of the Year.

Nominees must be submitted in a format similar to the following (whichever pertains to them):
John Smith / S/A / DEA / HIDTA 1
John Smith / TFO (Det.) / DEA HIDTA 1 (Edison PD)
John Smith / Det. / Edison PD / Narcotics Bureau

It must be completed and submitted no later than March 1st. 

The justification must also be emailed to editor@njneoa.org at the time of submission.
